The only way to fight the emptiness is by drinking/smoking. Thus, she escapes from reality and feels high, but after the effect of the opium is gone she feels really low and worse than before "been hurtin’ low from living high for so long”. This is the time when she realizes she is destroying herself "mine myself like coal" and regrets what she’s done. So she tries to change "just one last puff and two last regrets", “i promise this drink is my last one”. But are they really the last ones? No. So it starts all over again. Until she finds love. With him she doesn’t feel lonely anymore and doesn’t need drugs. But he is her only support and reason not to give up "my heart would break without you". He keeps her alive “might not awake without you”. But it’s like she knows she will fail one day and is scared she might lose him, so she apologises for the time when she will feel weak "i’m sorry and i love you", trying to keep him "don’t leave me, i’ll hate myself until i die".
