It's made to look like a part of her body. She's saying that the shoulder pads, high heels, emphasised cheekbones, eccentric makeup etc that she wears are all a part of who she really is. That's why she's made them all into realistic body parts as this exaggerated, alienesque type character. To show that she was born that way.

its because the shoes are her foot, not shoes, she was born with that foot
